Resumo: A violência doméstica contra a mulher é um problema de ordem social e de saúde pública, por ser cometida dentro do ambiente doméstico por pessoas com ou sem laço de consanguinidade é um dos fatores que a torna invisível para a sociedade e profissionais de saúde. As unidades básicas de saúde são fundamentais para a detecção destes casos de violência. OBJETIVOS: investigar a prevalência e fatores associados à violência doméstica entre mulheres usuárias de nove unidades de atenção básica de saúde de Maceió – Al. METODOLOGIA: estudo de corte transversal, realizado na cidade de Maceió – Al, em nove unidades de saúde: USF Vale do Reginaldo; Durval Cortez; Pitanguinha; Fernão Velho; Novo Mundo/José Araújo; Selma Bandeira, Rosanne Collor e Pescaria. Os critérios de inclusão foram: mulheres em atendimento nas unidades básicas de saúde ou no domicílio, com idade entre 18 e 49 anos, que tivessem parceiro íntimo na ocasião da entrevista e/ou residisse em domicílio com pai, padrasto, irmão ou agregado familiar e ter estado grávida entre 2014 e 2015. Foram excluídas da pesquisa mulheres em período gestacional, mulheres com limitações psicológicas ou emocionais que inviabilizavam a realização da entrevista e mulheres em relações homoafetivas. A amostra foi de 247 mulheres. O instrumento utilizado para detecção da violência foi o Abuse Assesment Screen (AAS). Na análise dos dados foi utilizado a estatística descritiva, Qui-quadrado e o Odds Ratio. RESULTADOS: entre as 247 entrevistadas, foi detectado prevalência de 43,72% de violência doméstica, 37,24% para a violência psicológica, 19,83% para a violência física e 8,09% para a violência sexual. Os principais agressores foram maridos e ex-maridos. Os tipos de agressões que prevaleceram para a violência física foram tapas, socos e chutes, nas regiões da cabeça, membros superiores e inferiores. A maioria das mulheres eram jovens, pardas, convivendo em união estável, com baixos níveis de escolaridade, desempregadas e com renda familiar de até um salário mínimo. Houve associação significativa entre violência física e renda familiar (p<0,005). A variável tabagismo se mostrou associada com as violências física (OR=3,202; IC95%=1,281-8,003) e psicológica (OR=3,298; IC95%=1,326-8,202) e o uso do álcool mostrou associação com a violência física (OR=2,533; IC95%=1,208-5,314). Ter presenciado violência na infância apresentou associação com violência física (OR=2,429; IC95%=1,279-4,614) e violência psicológica (OR=2,860; IC95%=1,675-4,873); Mulheres que presenciaram suas mães sendo agredidas pelos parceiros íntimos, também apresentaram maiores chances de sofrer violência física (OR=2,400; IC95%=1,271-4,529) e psicológica (OR=2,229; IC95%=1,309-3,794). Houve associação significativa com mulheres agredidas na infância com a violência do tipo psicológica (OR=1,899; IC95%=1,115-3,233). CONCLUSÃO: os dados mostraram alta prevalência de violência doméstica, em sua maioria praticada pelo parceiro íntimo. Dessa forma, se faz necessário maior investimento em capacitações com os profissionais das unidades básicas de saúde, para que possam reconhecer as situações de violência e lidar com esse fenômeno tão complexo e que abrange grande número de mulheres.
Abstract: Domestic violence against women is a problem of social order and public health, by being committed within the domestic environment for people with or without bond of consanguinity is one of the factors that makes it invisible to society and health professionals. Basic health units are essential to the detection of these cases of violence. OBJECTIVES: to investigate the prevalence and factors associated with domestic violence among women users of nine units of the basic attention to health of Maceió-Al. METHODOLOGY: this is a cross-sectional study, conducted in the city of Maceió-Al in nine health units: USF Vale do Reginaldo, Durval Cortez; Pitanguinha; Fernão Velho; Novo Mundo/Jose Araújo, Selma Bandeira, Rosanne Collor and Pescaria. Inclusion criteria were: women in attendance in basic health units or at home, aged between 18 and 49 years old, who had intimate partner on the occasion of the interview and/or residing at home with father, stepfather, brother or household and have been pregnant between 2014 and 2015. Women were excluded from research on gestational period, women with psychological or emotional limitations that not possible the completion of the interview and homosexual women. The sample was of 247 women. The instrument used for detection of violence was the Abuse Assessment Screen (AAS). In the analysis of the data was used the descriptive statistics, Chi-square and Odds Ratio. RESULTS: among the 247 interviewed, was detected 43.72% prevalence of domestic violence, 37.24% to psychological violence, 19.83% for physical violence and 8.09% for sexual violence. The main perpetrators were husbands and ex-husbands. The types of attacks that have prevailed for physical violence were slaps, punches and kicks, in the regions of the head, upper and lower limbs. Most of the women were young, Brown, living in stable, with low levels of education, unemployed and with household income of up to one minimum wage. There was a significant association between physical violence and family income (p < 0.005). The smoking variable proved to be associated with physical violence (OR=3,202, CI=1,281-8,003) and psychological (OR=3,298, CI=1,326-8,202) and alcohol use showed association with physical violence (OR=2,533, CI=1,208-5,314). Have witnessed violence in childhood showed association with physical violence (OR=2,429, CI=1,279-4,614) and psychological violence (OR=2,860, CI=1,675-4,873); Women who witnessed their mothers being abused by intimate partners, were also likely to suffer physical violence (OR=2,400, CI=1,271-4,529) and psychological (OR=2,229, CI=1,309-3,794). There was significant association with battered women in childhood with violence of psychological type (OR=1,899, CI=1,115-3,233). CONCLUSION: the data showed high prevalence of domestic violence in your most practiced by intimate partner. Thus, if necessary increased investment in training with the pros of the basic health units so that they can recognize the situations of violence and to deal with this phenomenon as complex and which covers a large number of women.
